Job Summary:

The Team Leader is an integral part of the support team, responsible for meeting the practical, physical, and emotional needs of the individuals we support within the service.

Through a key worker system, they will design individual support programmes and coordinate the delivery of these packages by a group of Support Workers and themselves.

About the role

Key Accountabilities

  • Plan, coordinate, and implement the safe transition of new individuals into the service and community as needed.

  • Communicate with the families of the individuals we support and relevant members of multi-disciplinary teams, community groups, and voluntary bodies to advocate for their needs.

  • Plan, develop, and implement individual and group support programmes to enhance and maximize the capabilities and independence of individuals in the service.

  • Coordinate regular assessments and reviews using appropriate assessment models.

  • Address the physical, emotional, and personal needs of individuals, ensuring that work is focused on individual needs and upholds principles of respect, privacy, dignity, fulfilment, independence, and choice.

  • Ensure that medication is held, stored, and administered according to Praxis Care’s Medication Policy and current statutory and legislative procedures.

  • Provide effective leadership, supervision, and management to Support Workers, including monitoring time and attendance and participating in capability, disciplinary, and grievance procedures as necessary.

  • Participate in organizational learning and development opportunities while identifying personal development opportunities for themselves and Support Workers.

  • Ensure that adequate records are created and maintained and that necessary reports are produced in compliance with organizational policy and statutory requirements.

  • Assist the manager with compliance concerning all relevant regulatory bodies.

  • Work the necessary hours to meet the needs of the service.

Essential and Desirable Criteria:

Education, Experience & Training Qualifications


Essential:

  • 2 years’ experience in a health and social care setting and currently enrolled in QCF Level 3 in Health and Social Care, OR

  • Understanding of the mental health/learning disability area, AND

  • Qualification in:

    • QCF/RCF Level 3 in Health and Social Care

    • Social Work

    • First Level Nursing

    • Allied Health Profession

    • Level 5 Diploma in Leadership for Health and Social Care (adults management/adults residential management, Wales & NI).

Desirable:

  • Experience in mental health or learning disability services.

  • Full valid driving licence and access to own vehicle (if a disability precludes an individual from holding a driver’s licence, it should be stated how they can meet the mobility requirement of the post).
