Reablement Professional Carer

Purpose of Position

To assist people in their own homes in retaining or regaining their skills and confidence, enabling them to manage daily tasks again after a period of illness or injury. Reablement supports discharge from the hospital, prevents readmission, or enables individuals to continue living at home. Reablement is typically provided for up to six weeks, though some individuals may require only a couple of weeks. Flexibility is essential to take on packages, assess clients' capacity on short notice, and implement aids to help them retain independence.


Principal Responsibilities

  • Assist with Mobility:Provide encouragement, support, and adaptations to help individuals regain confidence in moving around and being as independent as possible after injury or illness.

  • Making Food & Drinks:Empower and assist individuals in preparing their own food and drinks, helping where necessary.

  • Personal Care:Assist with personal care if an individual is unable to wash and dress independently, always preserving dignity and privacy and allowing them the freedom to do things at their own pace.

  • Housework:Encourage individuals to do as much as they can for themselves, exploring alternative methods to accomplish difficult tasks.

  • Shopping:Identify personal preferences and encourage individuals to continue with day-to-day tasks, such as shopping.

  • Family, Friends & Community Groups:Help individuals establish or maintain contact with family, friends, and community groups. Support their reconnection with the local community by providing information and building confidence to go out and use public transport.

  • Health & Wellbeing:Support independence and monitor health issues or deterioration, ensuring access to appropriate help when needed.

  • Pain Management:Ensure access to healthcare professionals for effective pain management.

  • Support Planning:Collaborate with healthcare professionals to assess care needs, aiding individuals in living as independently as possible once their reablement care concludes.
