Senior Carer and Support Worker

Support the Registered Care Manager to manage the Hunslet Moor House Extra Care service and work to manage a team of support workers deputizing in the absence of the Registered Manager.

To work alongside the Registered Manager to ensure the operational management, support and care is delivered to Hunslet Moor House in line with CQC requirements and to be responsible for the safe running of the service in the absence of the Deputy/Registered Manager and report any concerns/incidents to the Senior Management Team

Provide professional and operational day to day line management of a team of support workers within the Extra Care Service and the Outreach Care team.

To support the induction and provide ongoing support and leadership to a team of care and support workers whilst demonstrating the values and behaviours of Westward Care at all times

To assist in the management and formulation of rotas, duties and routines including the allocation of staff to planned care visits, scheduling of care calls and ongoing resident and support plan reviews.

To maintain adequate support for residents throughout the day and night and ensure that the appropriate cover is provided for outreach visits within the wider community.

Ensure the delivery of high-quality care and support of residents, service users and professionals to provide a responsive and integrated service which respects the diverse needs, lifestyle and aspirations of all those using the Service whilst promoting a person centered and flexible service which meets the needs of the individual.

Make referrals to Primary and Community Health Care Teams, Social Services, specialist services and agencies to ensure individual needs are identified and their health and wellbeing needs are met and maintained coordinating support service such as community nurses and care managers as required.

 Support the process of regular Quality Audits including Health and Safety Checks and Fire Precautions, to ensure that the companies policy on Health & Safety at Work is implemented.

 Be part of an ‘on call rota’ for the Hunslet Moor House extra care service

About the role

To support the Registered Manager at Hunslet Moor House to manage and work alongside a team of Care and Support Workers to ensure the smooth and efficient running and the day to day delivery of care and support as part of an Extra Care Service for Older People and in time a locality Outreach Service for vulnerable adults living in their own homes.

To provide the assessed level of care as determined by Westward Care and the Local Authority in collaboration with Preferred Home and other stakeholders, in line with Care Quality Commissions (CQC) Fundamental Standards of Quality and Safety and, ensure the highest level of customer service is provided at all times.

ADDITIONAL DUTIES

It is the nature of the work of Westward Care that tasks and responsibilities are, in many circumstances, unpredictable and varied.  Each staff member is, therefore, expected to work in a flexible way when the occasion arises, when tasks not specifically covered in their job description have to be undertaken.  Such additional duties would normally be to cover unforeseen circumstances or changes in work and would normally be compatible with the regular type of work.  If the additional responsibility or task becomes a regular or frequent part of the member of staff’s job, it will be included in the job description in consultation with the member of staff.

There May be occasions when you as a senior care and support worker will be asked to work in that capacity in one or other of our services including Care Home and Outreach Services.

CONFIDENTIALITY

It is expected that all Westward Care employees will understand that our work is confidential and that personal details must not be divulged to members of the public.  This obligation will continue indefinitely even after termination of employment.  All approaches by the media or other third parties must be referred, unanswered, to the line manager.
