At Compassion First Home Care, our mission is simple: Local care for local people.
We provide high-quality, person-centred care that enables people to remain independent, safe and comfortable in their own homes. As we continue to grow across South Warwickshire, we are looking for an experienced, organised and compassionate Care Coordinator to become a key member of our office team.
This is an important role at the heart of our service. You will be responsible for coordinating care visits, supporting our care team and helping ensure every customer receives safe, reliable and consistent care.
We are looking for someone who understands that great coordination isn't simply about filling a rota — it's about matching the right people, maintaining continuity and making sure our customers receive the standard of care they deserve.
About this role
What You'll Be Doing
As our Care Coordinator, you will work closely with the Registered Manager, Operations Director, Field Supervisors and wider care team to support the effective day-to-day running of the service.
Your responsibilities will include:
Coordinating and maintaining customer care schedules and staff rotas
Matching carers to customers based on needs, personality, location, skills and preferences
Promoting excellent continuity of care and reducing unnecessary changes of carers
Ensuring all customer visits are appropriately covered
Responding quickly and effectively to sickness, absence and changes in care requirements
Communicating rota changes clearly with carers, customers and families
Monitoring scheduled visits and responding to late, missed or potentially missed calls
Supporting new care packages and ensuring appropriate staff are allocated before services commence
Working closely with the recruitment team to identify current and future staffing requirements
Coordinating shadowing, introductions and competency requirements where appropriate
Maintaining accurate information within our electronic care planning and scheduling systems
Supporting the management of annual leave and staff availability
Helping ensure working hours, travel time and rotas are planned safely and realistically
Supporting customer reviews and responding to changes in care needs
Building positive relationships with customers, relatives, carers and healthcare professionals
Supporting the office with day-to-day operational and administrative duties
Participating in the on-call rota
Supporting the care team in the community when necessary
Escalating safeguarding, medication, health, staffing or service concerns appropriately
Supporting compliance with CQC regulations, company policies and best practice
Contributing to audits, quality monitoring and continuous service improvement
What We're Looking For
We are looking for someone who is organised, approachable and able to remain calm when priorities change.
Ideally, you will have:
Previous experience within domiciliary care, home care or a similar health and social care environment
Previous experience in care coordination, scheduling or a similar operational role
A good understanding of person-centred care
Strong organisational and time-management skills
Excellent telephone and face-to-face communication skills
Confidence dealing with customers, families and care professionals
The ability to manage competing priorities in a busy environment
Good IT skills and confidence using digital care planning and scheduling systems
A proactive approach to solving problems
A strong understanding of confidentiality and professional boundaries
A genuine commitment to providing high-quality care
Experience using Access Care Planning or similar care management systems would be advantageous but is not essential.
A Level 3 qualification in Health & Social Care or equivalent would also be desirable.
A full UK driving licence and access to a vehicle are preferred due to the community-based nature of our service.
The Person Behind the Role
We're not simply looking for someone who can manage a rota.
We want someone who understands the people behind it.
You'll need to recognise that changing a carer isn't just moving a name on a screen. For many of our customers, their carer is an important and trusted part of their daily life.
You'll be someone who communicates well, thinks ahead, takes ownership and genuinely cares about delivering a reliable service.
What We Offer
As part of the Compassion First Home Care team, you will receive:
A competitive salary
Supportive and approachable management
Workplace pension scheme
Holiday entitlement
FREE Blue Light Card, providing discounts with hundreds of retailers
Ongoing training and professional development
Opportunities to complete nationally recognised NVQ qualifications
Annual pay reviews based on cost of living
Genuine opportunities for career development and progression
The opportunity to play an important role in a growing, highly regarded local care provider
Why Compassion First?
We believe excellent care starts with excellent people.
We want our Care Coordinator to have the confidence and support to make good decisions, build strong relationships and help create a service where customers and carers feel genuinely valued.
