We are looking for a compassionate and caring Care Practitioner to join our team. This is a 9-month fixed-term contract to cover maternity leave, with the possibility of an extension. The role is due to start in mid-October 2026.
About this role
44 hours per week, 8:00am – 8:00pm
Working 4 shifts a week, with a requirement to work alternate weekends
Providing high-quality, person-centred care and supporting residents with all aspects of daily living, while promoting their dignity, independence and wellbeing.
Safely administering medication in accordance with prescribed instructions, policies and procedures.
Completing accurate and timely care records and all required documentation on a daily basis.
Supporting the induction, training and supervision of new members of the care team, sharing your knowledge and experience.
Completing, monitoring and reviewing residents’ care plans to ensure they remain accurate, current and responsive to changing needs.
Maintaining clear and comprehensive daily records, ensuring all relevant information is communicated effectively.
Monitoring and reporting on residents’ health, wellbeing and changing needs, escalating any concerns appropriately.
Liaising effectively with GPs, healthcare professionals, Support Managers and other relevant colleagues to ensure residents receive the appropriate care and support.
Working collaboratively with colleagues to maintain consistently high standards of care and contribute to a positive and supportive environment for residents and staff.

- Previous experience in a care home or working with the elderly is essential to be considered
- QCF/NVQ Level 3 in Health and Social Care
- Medication trained
