Job Summary:
Support Workers are central to Praxis Care’s mission of providing quality care to individuals. Each Support Worker is part of a dedicated team that delivers high-quality, reliable support services.
Within the Outreach service you would be required to provide support for adults with a physical disability and complex neurological conditions such as acquired brain injury. You will provide person centred support that focuses on outcomes, to enable each person we support to learn new skills and strategies to increase personal independence and facilitate access to community activities, education, and employment opportunities. One of the main objectives of this role is provide support that enables people to live as fully and independently as possible.

*Candidate note: This role is located on the Isle of Man and applicants must either be a resident or hold a work permit at time of appointment to take up the role*
All advertised salaries are based on 39 hours (FTE) and will be pro rata to contracted hours.**
About this role
Key Accountabilities:
Accountabilities:
· Support and empower individuals in the community to develop or improve independent living skills as identified in person-centred plans, which may include assistance with domestic, social skills, and personal care.
· Collaborate closely with Team Leaders and managers to prepare and implement individual and group support programs.
· Work with other Support Workers and Team Leaders to promote a safe working and living environment, effectively resolving conflicts or challenging behaviours using appropriate techniques and language.
· Ensure that medication is stored and administered according to Praxis Care’s Medication Policy when necessary.
· Familiarize yourself with all administrative requirements of the service, accurately recording and logging important information per reporting guidelines.
· Stay informed about all policies and procedures within the service (including Health & Safety), ensuring participation in all related training.
Requirements of the Role:
· Participate in the company appraisal and staff review process.
· Full valid driving license and access to own vehicle (If a disability precludes an individual from holding a driver’s license, they should state how they can meet the mobility requirement of the post).

· Carry out all duties in accordance with Praxis expectations, regulations, and policies.
· Be willing to take on additional responsibilities and duties as required.
· Work necessary hours to meet the needs of the service.
