We are seeking an experienced and compassionate Complex Care Nurse to join our dedicated community healthcare team, supporting an individual with significant and complex healthcare needs within their own home and community.

This is a hands-on clinical role requiring a confident and experienced nurse who can provide safe, consistent and person-centred care while supporting the wider care team.

About this role

Your Responsibilities

As a Complex Care Nurse, your responsibilities will include:

  • Delivering safe, effective and person-centred nursing care within the individual's home and community

  • Providing tracheostomy care and airway management

  • Managing and monitoring complex respiratory needs and chronic lung disease

  • Providing chest management and respiratory support

  • Recognising changes in respiratory presentation and responding appropriately

  • Managing and monitoring epilepsy and seizures

  • Managing gastrostomy feeding, hydration and medication administration

  • Supporting an individual who is nil by mouth

  • Safely administering and monitoring prescribed medications

  • Providing safe moving and handling support in line with the individual's care plan

  • Monitoring the individual's clinical presentation and escalating any deterioration or concerns appropriately

  • Maintaining accurate and professional clinical documentation

  • Following individualised care plans, risk assessments and clinical protocols

  • Providing clinical guidance and support to Healthcare Assistants working within the care package

  • Working collaboratively with families, carers and multidisciplinary healthcare professionals

  • Maintaining high standards of safeguarding, infection prevention and clinical governance

Essential Requirements

To be successful in this role, you will need:

  • Active NMC registration

  • Experience working as a Registered Nurse within complex care, community, acute or a similar clinical environment

  • Experience supporting individuals with tracheostomies and complex airway needs

  • Experience with respiratory and chest management

  • Experience with gastrostomy/enteral feeding and medication administration

  • Experience managing epilepsy and seizures

  • Experience supporting individuals with significant physical disabilities and complex moving and handling requirements

  • Strong clinical assessment and deterioration recognition skills

  • Confidence working within a community-based complex care package

  • Excellent communication and clinical documentation skills

  • A calm, professional and compassionate approach

  • Eligibility to work in the UK

Desirable Experience

Experience supporting individuals with any of the following would be advantageous:

  • Severe cerebral palsy

  • Significant developmental delay

  • Chronic lung disease

  • Tracheobronchomalacia

  • Complex tracheostomy and airway management

  • Gastro-oesophageal reflux (GOR)

  • Complex epilepsy

  • Non-verbal communication

  • Significant mobility and postural support needs

  • Community-based complex care

About the Individual You Will Support

You will be supporting an individual with significant neurological, respiratory and physical needs who requires skilled, consistent and attentive care.

The individual has a tracheostomy and requires ongoing airway and chest management due to complex respiratory needs. They also require support with epilepsy management, gastrostomy feeding and medication administration and are nil by mouth.

The individual is non-verbal, uses a wheelchair/P-Pod chair and requires full support with moving and handling on a 2:1 basis.

You will work as part of an established care team to ensure their health, safety, comfort, dignity and quality of life remain at the centre of all care provided.

What you will need

Required criteria

  • Registered Nurse with active NMC pin.
  • Minimum 2 years of post-registration experience within a complex care, community, or acute nursing setting.
  • Proven experience in at least one of the following: Tracheostomy care - Ventilation management - PEG / enteral feeding - Catheterisation and bowel management - Seizure management and medication administration
  • Experience in paediatric complex care (for children's caseloads).
  • Previous experience working in a homecare or community nursing role.
  • Strong leadership, risk assessment, and documentation skills.
  • Excellent communication and interpersonal abilities.
  • Full UK driving license and own vehicle (required for community visits).

Desired criteria

  • Clinical training qualification or mentorship experience.
