We are seeking an experienced and compassionate Complex Care Nurse to join our dedicated community healthcare team, supporting children and adults with complex health needs in their own homes and community settings.
This is a hands-on clinical role supporting individuals with significant and complex healthcare needs. You will play a vital role in delivering safe, person-centred care, overseeing clinical care plans and supporting Healthcare Assistants to maintain the highest standards of care.

About this role
Your Responsibilities
As a Complex Care Nurse, your responsibilities will include:
Delivering safe, effective and person-centred nursing care to individuals with complex health needs
Managing and monitoring tracheostomy care, including airway management and secretion management
Providing chest management and respiratory support
Managing and monitoring epilepsy and complex seizure presentations
Supporting individuals with insulin-dependent diabetes
Managing gastrostomy feeding, hydration and medication administration
Safely administering and monitoring prescribed medications
Supporting individuals who are nil by mouth
Providing safe moving and handling support for individuals with significant physical disabilities
Monitoring for changes or deterioration in an individual's clinical presentation and escalating concerns appropriately
Following individualised care plans, risk assessments and advanced care plans
Maintaining accurate and professional clinical documentation
Providing clinical guidance and support to Healthcare Assistants working within the care package
Working closely with families, carers and multidisciplinary healthcare professionals to ensure continuity of care
Maintaining high standards of safeguarding, infection prevention and clinical governance
Essential Requirements
To be successful in this role, you will need:
Active NMC registration
Experience working as a Registered Nurse within a complex care, community, acute or similar clinical environment
Experience supporting individuals with tracheostomies and complex respiratory needs
Experience with gastrostomy/enteral feeding and medication administration
Experience managing epilepsy and seizures
Experience supporting individuals with significant physical disabilities and complex moving and handling requirements
Strong clinical assessment and deterioration recognition skills
Confidence working independently within a community setting
Excellent communication and clinical documentation skills
A calm, professional and compassionate approach

Desirable Experience
Experience in any of the following would be advantageous:
Insulin-dependent diabetes management
Complex neurological conditions
Hypoxic brain injury
Epileptic encephalopathy
Spastic quadriplegia and dystonia
Complex respiratory and chest management
Supporting non-verbal individuals with complex communication needs
Advanced care planning
Community-based complex care
About the Individual You Will Support
You will be supporting an individual with significant neurological, respiratory, mobility and clinical needs who requires a high level of skilled and consistent care.
The individual is non-verbal, uses a wheelchair and requires full support with moving and handling. Their care includes tracheostomy and chest management, seizure management, diabetes management, gastrostomy support, medication administration and ongoing monitoring of their complex health needs.
You will work as part of an established care team to ensure their health, safety, dignity and quality of life remain at the centre of all care provided.

What you will need
Required criteria
- Registered Nurse with active NMC pin.
- Minimum 2 years of post-registration experience within a complex care, community, or acute nursing setting.
- Proven experience in at least one of the following: Tracheostomy care - Ventilation management - PEG / enteral feeding - Catheterisation and bowel management - Seizure management and medication administration
- Experience in paediatric complex care (for children's caseloads).
- Previous experience working in a homecare or community nursing role.
- Strong leadership, risk assessment, and documentation skills.
- Excellent communication and interpersonal abilities.
- Full UK driving license and own vehicle (required for community visits).
Desired criteria
- Clinical training qualification or mentorship experience.
