Summary of role
Ensure all client visits are allocated, recruit suitable Care and Support Workers, and maintain appropriate office procedures.

What the Care Coordinator role involves?
Organisation of rotas by liaising with our care team
Managing new customer referrals, following up enquiries and converting opportunities into new care packages
Onboarding new customers and ensuring care is set up smoothly from the start
Liaising with customers, families and care staff to ensure care needs are met and changes are responded to promptly
Maintaining accurate care records and ensuring information is kept up to date
Providing exceptional customer service for clients and care teams
Assist with on call duties

1. You must have great communication skills and be able to sell our service on the phone
2. A quick learner
3. Excellent organisation skills is also a must
4. You should have proficient IT skills as rotas and care plans are updated using our IT systems
5. A team player
The role is based in our Medway office and is a full time position.
