Complex Care Assistant – Nurse-Led Care

As a Complex Care Assistant, you will be assigned to one or two clients on a consistent basis (or work flexibly across a range of packages as a relief worker), delivering high-quality, person-centred care in the client's own home. Your day-to-day responsibilities will include:

  • Delivering clinical care tasks as trained and signed off by our nursing team, which may include gastrostomy (PEG) feeding, tracheostomy care, oxygen/ventilation management, catheter care, and bowel/stoma care

  • Administering medication in line with the client's care plan and Third Hand Care policy

  • Supporting personal care, mobility, and daily living activities

  • Monitoring and accurately recording the client's condition, escalating any changes promptly to the clinical lead or nurse

  • Following individual care and support plans, risk assessments, and behaviour support plans where applicable

  • Working collaboratively with family members, other care staff, and visiting healthcare professionals as part of a coordinated care team

  • Maintaining accurate, timely care documentation in line with CQC requirements

  • Attending mandatory and specialist clinical training, supervision, and competency assessments

  • Upholding the client's dignity, independence, and choice at all times, adapting your approach to fit within the family home environment

  • Working shifts on a block basis (typically [4–12] hours), which may include days, nights, evenings, and weekends on a 7-day rota

About the Role

We are looking for compassionate, skilled care professionals to join our nurse-led complex care team. This is an opportunity to build on your care experience and develop specialist clinical skills, supporting clients with more complex, long-term health needs in their own homes.

You'll receive dedicated training and ongoing supervision from qualified nurses, working towards competencies in areas such as:

  • PEG feeding and gastrostomy care

  • Tracheostomy care

  • Oxygen and ventilation support

  • Catheter, bowel and stoma care

  • Medication administration

  • Personal care and behaviour support

Who You'll Support

Our complex care clients include children, young people, adults and older people living with a range of conditions — including neurological conditions such as MS and motor neurone disease, physical disabilities, learning disabilities, and acquired brain or spinal injuries following stroke or trauma. Packages are typically 1:1, with some 2:1 or 3:1 support for higher-need clients.

What We're Looking For

  • Previous experience in a care role (professional or as a family carer)

  • A full UK driving licence, essential

  • Flexibility to work varied shifts, including nights and weekends

  • A patient, empathetic and professional approach when working within a family home

  • Good spoken and written English, with strong communication skills

  • A genuine commitment to high-quality, person-centred care
